FHA Single Family Residential (Form 1004)

For FHA loans, a comprehensive Uniform Residential Appraisal Report of a single-family home, including an interior inspection by an FHA-certfied appraiser using form 1004, with an FHA addendum.

Single Family Residential (Form 1004)

For appraisals of one-unit properties and units in PUDs based on interior and exterior property inspections. In addition, appraisals for units in condo projects that consist solely of detached dwellings may be documented on Form 1004, if the appraiser includes an adequate description of the project and information about the homeowners’ association fees and the quality of the project maintenance. Appraisals reported on Form 1004 must be completed in accordance with the UAD Specification.

Exterior-Only Inspection Residential (Form 2055)

For appraisals of one-unit properties and units in PUDs based on exterior-only property inspections. Appraisals reported on Form 2055 must be completed in accordance with the UAD Specification.

Condominium Appraisal (Form 1073)

For appraisals of one-unit properties in condo projects based on interior and exterior property inspections. Appraisals reported on Form 1073 must be completed in accordance with the UAD Specification.

Exterior-Only Inspection Condominium Appraisal (Form 1075)

For appraisals of one-unit properties in condo projects based on exterior-only property inspections. Appraisals reported on Form 1075 must be completed in accordance with the UAD Specification.

Small Residential Income Property Appraisal (Form 1025)

For appraisals of two- to four-unit properties (including two- to four-unit properties in PUD, condo, or co-op projects) based on interior and exterior property inspections.

Manufactured Home Appraisal (Form 1004C)

For appraisals of one-unit manufactured homes (including manufactured homes in a PUD, condo, or co-op project) based on interior and exterior property inspections.

Desk/Enhanced Desk Review (Form 1033)

Desk Appraisal Reviews are completed for both pre-funding and post-funding audits. Confirmation of the data utilized in the report is made from a variety of publicly available sources. Any additional data considered by the reviewer is subsequently reported within the body of the completed review.

One-Unit Residential Appraisal Field Review (Form 2000)

A lender uses this form for the spot-check appraisal component of its quality control process. This appraisal field review report is intended to provide the lender/client with an opinion on the accuracy of the appraisal report under review.

Appraisal Update and/or Completion Report (Form 1004D)

For appraisal updates and/or completion reports for all one- to four-unit appraisal reports.

Rental Survey (Form 1007)

The Single Family Comparable Rent Schedule Form 1007 is intended to provide an appraiser with a familiar format to estimate the market rent of a property. Adjustments should be made only for those items of significant difference between the comparables and the subject property.

Operating Income Statement (Form 216)

Applicant provides income and expense projections to be used in determining income approach to value.
